Prescriptions, registration forms, surgical clearance and where we operate.

Managing prescriptions and renewals

For continuity of care, all prescriptions and authorizations should be requested during regular office hours. Please note that renewals requested outside of these hours will only be filled under extreme or emergent circumstances.

Completing patient registration forms

Before your appointment, you will fill out a Patient Registration form. These forms, available in both English and Spanish, help us understand your medical history and provide you with the most appropriate care.

Complete your forms before you arrive

Pre-operative surgical medical clearance

Depending on your age and medical history, you may be required to obtain medical clearance before surgery. This can be done no less than one week prior to surgery. The clearance process may include a history and physical, blood tests, EKG, and a chest X-ray.
